An outreach case management, transitional and reintegration service for all genders with a history of involvement in the criminal justice system and at risk of homelessness who are looking to live in the Penrith and Blue Mountains Region.
The Nepean/Blue Mountains project works with people on release from prison who are homeless, or at risk of homelessness and who are looking to live in the Nepean, Penrith or Blue Mountains region. Transitional workers offer pre-release support and planning, and short, medium and long term intensive holistic case management (including assistance with housing).
Priority for this service is given to people who are currently incarcerated.
Wherever possible referrals should be made at least three months prior to release.
This project is a partnership with Wentworth Housing and is funded through DCJ.
Referral Process
Corrective Services staff wishing to make a referral can access a referral form here. This form should then be emailed to sydney.transition@crcnsw.org.au. Please contact CRC directly on (02) 9288 8700 or through our contact us page if you need more information.
People in prison who would like to be referred to the service should contact relevant staff in their Correctional Centre. This might be a SAPO, a Parole Officer or a case manager. It is possible to self-refer by writing to CRC directly – address details are on our contact us page.
